Developmental Disability Support

Living at home can still be an option when you have or support someone with a mental or developmental disability.  Our CARE Professionals blend in with your family while providing companionship, personal care and housekeeping support to adults with disabilities.   

Personal Care

  • Assistance with bathroom activities (incontinence, inconsistency, urgency accidents).
  • Personal hygiene (hair care, skin care, oral hygiene).
  • Dressing/Grooming (dressing, assistance with personal grooming).
  • Assistance with mobility (walking, range of motions, light exercises).

Meal Preparation

CARE Connection's meal preparation services and CARE Professionals can create nutritious and delicious meals for breakfast, lunch and dinner that you and your loved one will enjoy.  Our dietitians and nutritionists are here to help you plan delicious, healthy meals.  

Medication Reminder

Our CARE Professionals are careful to ensure that you or your loved on is taking the correct medication at the appropriate time of day.  Some of the medication management services we offer include:  

  • Ensure medication is taken in the proper dosage at the proper time.
  • Setting up the pill box to help with managing medication.
  • Logging medication intake daily. 

Hospital Discharge

We provide free same day consultation before discharge by our gerontologists, social workers or nurses.  We will create a customized care plan to your specific needs of care.  You can interview potential CARE Professionals at the hospital to help you transition in your home.  


  • Following discharge orders
  • Prescription pickup, grocery shopping & other errands
  • Meal preparation 
  • Transportation to your home
  • Communication with healthcare providers and loved ones

Companionship
